چکیده

Several therapeutic options have been proposed for patients with recurrent or persistent post-prostatectomy urinary incontinence due to urethral atrophy: changing the balloon reservoir for a higher pressure one, downsizing the cuff diameter, or increasing the amount of fluid in the system (1-3). Theoretically, a transcorporal cuff could possibly provide some supplementary bulk of tissue to the circumference of the urethra, possibly decreasing the risk of erosion (4). This article Linder, et al. (5) deals with a matter of great clinical interest. This is a retrospective series reporting 69 cases of revision surgeries for urethral atrophy, of which 56 (82%) underwent tandem cuff placements, 12 (18%) underwent single cuff downsizings and one case had a single cuff relocated proximally. There was no difference in 3-year overall device survival compared between single cuff and tandem cuff revisions (60% vs. 76%, p=0.94). Of the 56 tandem cuff placements, 8 (14%) were performed with transcorporal approach. Interestingly, these patients had adverse 3-year device survival compared to those without a transcorporal approach (44% vs. 80%, p=0.0016). Despite of the inherent limitations of this retrospective study, it seems that a transcorporal approach should be reserved for very selected patients (most probably in secondary or tertiary interventions). Randomized controlled trials are still needed to guide what is the best technique for each group of patients, taking into account anatomical characteristics, previous radiation therapy, the risk of urethral erosion and other local complications. Artificial urinary sphincter revision for urethral atrophy: comparing single cuff downsizing and tandem cuff placement

OBJECTIVE To compare outcomes for single urethral cuff downsizing versus tandem cuff placement during artificial urinary sphincter (AUS) revision for urethral atrophy. MATERIALS AND METHODS We identified 1778 AUS surgeries performed at our institution from 1990-2014. Of these, 406 were first AUS revisions, including 69 revisions for urethral atrophy. Multiple clinical and surgical variables w...

Artificial urinary sphincters for male stress urinary incontinence: current perspectives

The artificial urinary sphincter (AUS), which has evolved over many years, has become a safe and reliable treatment for stress urinary incontinence and is currently the gold standard. After 4 decades of existence, there is substantial experience with the AUS. Today AUS is most commonly placed for postprostatectomy stress urinary incontinence. Only a small proportion of urologists routinely plac...

Perineal approach for artificial urinary sphincter implantation appears to control male stress incontinence better than the transscrotal approach.

PURPOSE Traditionally cuff placement of an artificial urinary sphincter is done through a perineal approach. A new approach through a penoscrotal incision or transscrotal approach is reportedly more rapid and easier than the traditional incision. These 2 approaches were evaluated to determine which one controlled male stress urinary incontinence better. MATERIALS AND METHODS We performed a re...

A multicenter study on the perineal versus penoscrotal approach for implantation of an artificial urinary sphincter: cuff size and control of male stress urinary incontinence.

PURPOSE In a single center retrospective study we previously reported superior dry rates and fewer artificial urinary sphincter revisions when the sphincter cuff was placed via the traditional perineal approach compared with a penoscrotal approach. A multicenter study was performed to compare the approaches further and explain the disparity in outcomes. MATERIALS AND METHODS We performed a re...

In situ urethroplasty after artificial urinary sphincter cuff erosion

Artificial urinary sphincter (AUS) cuff erosion is a challenging complication traditionally managed with device removal and Foley catheter drainage. Urethral stricture can result secondary to the healing process, delaying AUS reimplantation. In situ urethroplasty (ISU) technique is a definitive repair at the time of device removal. Early results demonstrate a decreased rate of stricture formati...
